Print the raffle ticket below by clicking on the printable version link. Fill out the necessary information on the lines that say your name, address, etc. Cut the raffle ticket in half along the line. Fill out a check of $10 payable to Hope For Haiti's Children Inc.. Send the portion of the raffle ticket that includes your name, address, etc. and check to : Hope For Haiti's Children Inc. 16477 SW 100th Terrace Miami, FL 33196 Once we receive your check and ticket you will be notified by email or phone. If you win a prize, it will be shipped at no extra charge to the address you provide. |
